Thank you for entrusting the care
of, and any necessary treatment for, your pet to Andale Veterinary
Centre. Here are details of our Practice Terms and Conditions.
Some aspects of the Terms may not be relevant to you. Do ask for
further explanation or clarification if required.
Estimates of Treatment Costs
We will be pleased to provide a
written estimate as to the probable costs of any treatment. Any
estimate can only be approximate as it may be that illness will
not follow a conventional course.
Fees
All fees, diets and drug charges
are subject to VAT at the current rate. Fee levels are determined
by the time spent on each case and according to the drugs,
materials, consumables and diets used. Our written fee list is
available on request. You will receive a detailed fee note for
every consultation, surgical procedure or transaction with us.
Methods of Payment
All accounts are due for
settlement at the end of the consultation; on the discharge of
your pet; or upon collection of drugs; diets; or purchases from
our shop. You may settle the account using one of the following:
Cash, credit / debit card, MasterCard, Visa, Switch, Solo, Delta.
Settlement Terms
Please settle your account
promptly. Should an account not be settled within 7 days, a
reminder will be sent. Should the account not be settled in
response to the reminder then a further letter will be sent and an
administration charge of £15 will be imposed if the account is
not paid within a further 7 days. If the account remains
outstanding, the administration charge will be added to the
account and a final letter will give 5 days to pay the outstanding
debt. Thereafter all outstanding debts will be referred to our
Debt Collection Agency and further charges may apply i.e.
production of reports, correspondence, court fees, attendance at
court, phone calls, home visits etc.
Any cheque returned by our Bank as unpaid, any Credit Card payment
not honoured and any Cash tendered that is found to be counterfeit
will result in any bank charges and administration costs being
added to the original account, and where appropriate, interest on
the principal unpaid sum.
Inability to Pay
If for any reason you are unable
to settle your account as specified we ask that you discuss the
matter as soon as possible with a member of staff. Only the
Practice Directors may sanction instalments or part-payments of an
account.
Pet Health Insurance
Andale Veterinary Centre strongly
supports the principle of insuring your pet against unexpected
illness or accidents. Please ask for details about insurance from
any member of staff. Remember that it will be your responsibility
to settle your account with us and to then reclaim the fees from
the insurance company. We will help you to the best of our
ability.
Complaints and Standards
We strive to ensure that you
never have cause to complain about the standards of service you
receive from Andale Veterinary Centre. However, if you feel that
you have a legitimate complaint we ask that you speak in the first
instance to a member of the reception team. If the matter is not
resolved to your satisfaction do not hesitate to contact either of
the Practice Directors who will give the matter personal
attention.
Ownership of Records
Case records and associated
documents including radiographs are the property of, and will be
retained by, Andale Veterinary Centre. Copies, with a summary of
the history, will be passed on request to another veterinary
surgeon taking over the case.
Ownership of Radiographs and
Similar Records
The care given to your animal may
involve making some specific investigations, for example
radiographs or ultrasound scans. A charge is made for carrying out
these investigations and interpreting results however ownership of
the resulting record, for example a radiograph remains with Andale
Veterinary Centre.
Additions and Alterations
No addition to or variation of
these conditions will bind the practice unless specifically agreed
in writing and signed by either of the Practice Directors. No
agent or person employed by or under contract with the practice
has the authority to alter or vary these conditions.
